You will need the following documents:

Consolidated statement of income (Sample) / Notice of Assessment for year of assessment 2020 submitted to IRAS in capacity of SEP (Sample)

If you don’t have the Consolidated Statement of Income / Notice of Assessment for year of assessment 2020 submitted to IRAS in capacity of SEP, or if your trade income < employment income, you will need to produce the following documents for appeal:

Contract for Service should include:

  • UEN registration number of applicant’s company with Letterhead
  • Name and NRIC of applicant
  • Date of the service provided
  • Endorsement by company (client’s company stamp) and signed by both parties

Invoice or Bank Statement
to prove delivery and payment of service

e2i-4._CPF_Statement_of_Account

You will have 30 minutes to complete the Form once you login with your SingPass.

Additionally, please provide your email address in the form so that you can receive an email acknowledgement after submitting your application.

For appeal cases, please note that as each situation is unique, applications are subjected to approval on a case-by-case basis.

Once your application is approved, you will receive a copy of the Letter of Eligibility from NTUC’s e2i within 10 working days.
